What is Bronchiectasis?

Bronchiectasis is the permanent enlargement of the bronchi after damage to the bronchial wall.

Generally, bronchial damage occurs due to childhood infections (such as measles and pertussis). It manifests itself as copious sputum production, frequent respiratory tract infections, weakness and weakness in patients.

What are the Treatment Methods?

  • It is primarily treated with antibiotics and inhalers.
  • By applying the postural drainage method, the bronchial cleansing of the patients is ensured.
  • With respiratory physiotherapy, the lungs are strengthened and their capacity is increased.
  • The other treatment method is surgical removal of the diseased lobe.
  • Lung transplantation is necessary if both lungs are sick.

Why Do I Have to Have Surgery?

In cases where it is not possible to treat with drugs, the diseased area should be surgically removed.

Surgery is performed in cases where bronchiectasis is located in a restricted area of the lung. Surgical treatment is not applied in cases controlled by drug treatments or in diffuse bronchiectasis.

In advanced cases, if there is no response to drug treatment and quality of life deteriorates, if there are conditions such as bleeding from the mouth or inflammation in the lung cavity and frequent hospitalizations, surgical intervention is required.

One of the most important points here is the deterioration of the patient’s quality of life. If the patient has a bronchiectasis disease that affects his social life, surgical intervention is beneficial.

Quality of life improves significantly after bronchiectasis operations.

What Do International Sources Recommend?

It is stated that surgical treatment should be applied in cases where drug treatments do not achieve successful results, in cases where the complaints of the patients are high, there are infections so frequent that the patient needs to use drugs continuously, and there is a large amount of blood coming from the mouth with coughing.

What are the surgical methods?

In the open method, the lungs are reached with an incision of approximately 15 cm made between the 5th and 6th ribs. The affected lobe or lung segment is removed after its vessels and bronchus are cut and sutured. If there is an air leak from the remaining lung tissue, these are repaired and the procedure is terminated by placing 1 or 2 drains.

In the closed (endoscopic) method , 2 incisions of 1 cm are made. A camera and additional surgical instruments are advanced from here. A 5 cm auxiliary incision is made and the diseased lung lobe is removed without expanding the ribs. In the same way, the process is terminated after the air leaks are repaired and the drain is placed.

What Are the Risks of the Surgery?

There are risks such as post-operative bleeding, pneumonia, infection, and air leakage from the lungs. These conditions can be seen in 5-7% of patients.

How Many Days Should I Stay in the Hospital for Surgery?

On average, patients are discharged 4-5 days after surgery.
